Immunization Policy

 

California immunization requirements for elementary school students are as follows:

 

Number of Doses Required for Each Immunization:

4 Polio

5 DTaP

3 Hep B

2 MMR

2 Varicella

 

DTap/Tdap = diphtheria toxoid, tetanus toxoid, and acellular pertussis vaccine

Hep B = hepatitis B vaccine

MMR = measles, mumps, and rubella vaccine

Varicella = chickenpox vaccine

Special Exemptions: There are some special exemptions (waivers) allowed by the State (medical and/or personal beliefs). Personal Belief Exemptions will not be granted after 12/31/2015 per state law. These exemptions will be approved when the appropriate forms are completed.
